Este perfil representa as restrições aplicadas ao recurso AllergyIntoleranceIPS do Guia de Implementação FHIR do Sumário nternacional do Paciente (IPS).

Ele restringe a representação do registro de uma alergia ou intolerância do paciente, no contexto do resumo internacional do paciente, conforme especificado pelo projeto do International Patient Summary (IPS)em colaboração com HL7 Internacional.

O projeto INSPIRA (Hospital Sírio-Libanês / HL7 Brasil, Protocolo 25000.176202/2023-57) revisou os perfis do Guia IPS Brasil, adicionou bindings nacionais da RNDS e expandiu o mapeamento de estrutura para 8 colunas. A versão atual do Guia é 1.1.0 (2026).

Escopo/Uso

O perfil AllergyIntoleranceBRIPS especializa o perfil internacional AllergyIntolerance-uv-ips para o contexto brasileiro, mapeando o perfil RNDS BRAlergiaReacaoAdversa-1.0 (RAC/SA). Um registro de alergia ou reação adversa é representado no sumário do paciente como uma instância do recurso AllergyIntolerance restringida por esse perfil.

O campo AllergyIntolerance.code é obrigatório (1..1, MustSupport) e deve ser preenchido com o alergeno. O binding, herdado do IPS internacional, é de força preferred e aponta para o ValueSet absent-or-unknown-allergies-uv-ips, que inclui conceitos SNOMED CT GPS além de códigos para ausência ou desconhecimento de alergias. A RNDS utiliza o ValueSet nacional BRAlergenos-1.0 (required), que contém os alérgenos padronizados pela ANVISA e pelo Ministério da Saúde (RDC nº 204/2017 e Portaria MS nº 1.660/2009). O BRIPS mantém o binding IPS (preferred) com additional-bindings para conformidade internacional.

​O campo AllergyIntolerance.reaction.manifestation é obrigatório (1..*) e deve ser codificado conforme o ValueSet allergy-reaction-uv-ips (SNOMED CT GPS, preferred) — diferente do binding RNDS que usa BRReacoesAdversasMedDRA-1.0 (MedDRA, required). A extensão abatement-dateTime-uv-ips está disponível para informar a data de resolução da alergia (0..1).

Uso indevido

O recurso AllergyIntolerance não deve ser utilizado para registrar efeitos adversos de medicamentos que se configurem como diagnósticos (use ConditionBRIPS), nem para registrar intolerâncias alimentares de natureza nutricional sem relevância clínica imediata. Não deve ser usado para registrar eventos adversos pós-vacinação — para isso utilize o recurso AdverseEvent referenciando o ImmunizationBRIPS correspondente.

Extensões

• abatement-dateTime-uv-ips (0..1) — extensão herdada do perfil IPS internacional para registrar a data/hora de resolução (abatement) da alergia ou reação adversa. A RNDS e o br-core não declaram esta extensão; o BRIPS a mantém por herança do IPS parent.

Limites e Relacionamentos

O recurso AllergyIntoleranceBRIPS é referenciado pela seção “Alergias e Intolerâncias” (LOINC 48765-2) da CompositionBRIPS (section.entry), e também pode ser referenciado por AdverseEvent como instância suspeita do evento adverso (suspectEntity.instance).

O campo AllergyIntolerance.patient deve referenciar exclusivamente o perfil PatientBRIPS. Os campos recorder e asserter especializam as referências para os perfis BRIPS (PractitionerBRIPS, PractitionerRoleBRIPS, PatientBRIPS ou RelatedPerson).

O campo AllergyIntolerance.identifier é mantido como 0..* no BRIPS, ao contrário do BRAlergiaReacaoAdversa-1.0 que o zera (0..0). A cardinalidade canônica IPS é preservada pois o sumário histórico pode agregar registros de múltiplas fontes. O campo AllergyIntolerance.verificationStatus mantém o binding canônico (allergyintolerance-verification, required), diferente da RNDS que aplica BRGrauCertezaAlergiasReacoesAdversas-1.0.

Condição de entrada

O documento do IPS extrai e exibe todas as informações de Alergia e Intolerância registradas no Perfil BRAlergiaReacaoAdversa-1.0, presente nos seguintes documentos RNDS:

• RAC — Registro de Atendimento Clínico
• SA — Sumário de Alta Hospitalar
• SAO — Sumário de Alta Obstétrico

O Perfil da RNDS foi mapeado para o recurso AllergyIntoleranceBRIPS conforme mapa de estrutura descrito no documento de casos de uso.

O registro de alergia ou intolerância no IPS Brasil detalha o tipo de reação (erupção cutânea, anafilaxia), os agentes causadores, a categoria (alimento, medicação, ambiente ou outras substâncias), o tipo de alergia e, opcionalmente, a criticidade e certeza da alergia. Outros tipos de reações (por exemplo, desencadeadas por estímulos físicos como luz, calor, frio, pressão ou vibração) devem ser registradas como Condition na lista de problemas, e não usando o recurso AllergyIntolerance.

Condição de saída

As informações de alergia e intolerância exibidas no Sumário Internacional do Paciente correspondem a todos os registros RNDS de AllergyIntolerance cujo Status clínico (clinicalStatus) esteja codificado como ativo.

As alergias e intolerâncias oriundas de Sumários internacionais são exibidas exatamente como recebidas.

Mapeamentos terminológicos RNDS — IPS

Os CodeSystems e ValueSets do Perfil da RNDS utilizam os mesmos perfis do IPS, à exceção do ValueSet BRMedDRA, usado no elemento AllergyIntolerance.reaction.manifestation do Perfil da RNDS, que deve ser mapeado para o ValueSet allergy-reaction-snomed-ct-ips-free-set (preferred) do Perfil IPS.
