Este perfil representa as restrições aplicadas ao recurso Medication pelo Guia de Implementação do FHIR do Resumo Internacional do Paciente (IPS). Um medicamento é descrito no sumário do paciente como uma instância de um recurso Medicamentos restrito por esse perfil

O projeto INSPIRA (Hospital Sírio-Libanês / HL7 Brasil, Protocolo 25000.176202/2023-57) revisou os perfis do Guia IPS Brasil, adicionou bindings nacionais da RNDS e expandiu o mapeamento de estrutura para 8 colunas. A versão atual do Guia é 1.1.0 (2026).

Escopo/Uso

O perfil MedicationBRIPS especializa o perfil internacional Medication-uv-ips para o contexto brasileiro, mapeando o perfil RNDS RNDSMedicamento (baseDefinition: br-core-medication). O recurso Medication é usado como recurso referenciado (não como entry independente do Bundle IPS) para detalhar o medicamento em uso registrado em MedicationStatementBRIPS ou MedicationRequestBRIPS.

​O campo Medication.code é obrigatório na RNDS (1..1) e deve ser preenchido com o código do medicamento. O binding IPS é de força preferred e aponta para o ValueSet medication-code-uv-ips, que inclui SNOMED CT GPS e Absent/Unknown. A RNDS utiliza o ValueSet nacional BRTerminologiaMedicamento (required), construído a partir de múltiplos CodeSystems da OBM (BRObmCATMAT, BRObmEAN, BRObmANVISA, BRObmAMPP, BRObmVMP). O BRIPS mantém o binding IPS (preferred) por compatibilidade internacional; a RNDS e o br-core não declaram binding explícito no code do br-core-medication.

Nota — OBM em atualização: Os ValueSets baseados na OBM (BRTerminologiaMedicamento, BRUnidadeMedidaMedicamento e CodeSystems relacionados) estão em revisão no projeto INSPIRA. Os bindings nacionais documentados neste perfil refletem o estado atual da RNDS e serão atualizados em versão futura do Guia IPS Brasil à medida que a OBM revisada for publicada.

O campo Medication.form (forma farmacêutica) é obrigatório na RNDS (1..1, required BRUnidadeMedidaMedicamento) mas opcional no IPS (0..1, example medicine-doseform — EDQM). O campo Medication.ingredient.item[x] deve referenciar substâncias codificadas conforme o ValueSet medicine-active-substances-uv-ips (SNOMED CT GPS, preferred) no IPS.

Uso indevido

O recurso Medication não deve ser usado para registrar vacinas ou imunobiológicos (use ImmunizationBRIPS), nem para registrar dispositivos médicos (use DeviceBRIPS). Não deve ser utilizado diretamente como entry do sumário IPS — deve ser sempre referenciado por MedicationStatementBRIPS ou MedicationRequestBRIPS.

Limites e Relacionamentos

O recurso MedicationBRIPS é referenciado por:

  • MedicationStatementBRIPS — em medication[x] (medicamentReference) como o medicamento em uso.
  • MedicationRequestBRIPS — em medication[x] (medicamentReference) como o medicamento prescrito.
  • OrganizationBRIPS — referenciado POR ele em manufacturer (fabricante do medicamento).
  • Substance / MedicationBRIPS — referenciados POR ele em ingredient.item[x] (substância ativa ou medicamento componente).
  • CompositionBRIPS / BundleBRIPS — incluído no documento IPS como Bundle.entry e referenciado pelas seções de medicação via medicationReference.

Caso de Uso

Condição de entrada

A StructureDefinition BRMedicamento, utilizada para identificar e detalhar a composição dos medicamentos prescritos na RNDS, foi mapeada para o Perfil MedicationBRIPS.

Para identificação dos códigos do medicamento existem três possibilidades:

  1. Conjunto de conceitos codificáveis da SNOMED CT (Medications — SNOMED CT IPS Free Set);
  2. Sistema de classificação anatômica da OMS (WHO ATC — IPS);
  3. Código indicando ausência ou falta de conhecimento sobre o medicamento do paciente (Absent or Unknown Medication — IPS).

No caso dos dados extraídos da RNDS, são utilizados os identificadores da base nacional de medicamentos OBM — Ontologia Brasileira de Medicamentos, bem como o respectivo código WHO-ATC-IPS quando disponível. A OBM está sendo revisada no projeto INSPIRA; os mapeamentos terminológicos abaixo serão atualizados quando a nova versão for publicada.

Os principais elementos incluem: code (BRTerminologiaMedicamento + ATC), form (BRUnidadeMedidaMedicamento), status, batch.lotNumber e batch.expirationDate.

Condição de saída

O recurso Medication é documentado no Sumário Internacional do Paciente BR como um recurso referenciado (não como entry independente), sendo referenciado a partir do MedicationStatement ou MedicationRequest.
