O projeto INSPIRA (Hospital Sírio-Libanês / HL7 Brasil, Protocolo 25000.176202/2023-57) revisou os perfis do Guia IPS Brasil, adicionou bindings nacionais da RNDS e expandiu o mapeamento de estrutura para 8 colunas. A versão atual do Guia é 1.1.0 (2026).

Escopo/Uso

O perfil ObservationResultsLaboratoryBRIPS especializa o perfil internacional Observation-results-laboratory-uv-ips para o contexto brasileiro, mapeando o perfil RNDS BRDiagnosticoLaboratorioClinico (vigente, via documento REL — Resultado de Exame Laboratorial). O escopo desta seção no BRIPS é exclusivamente laboratorial — resultados de Patologia e Radiologia estão fora do escopo, e o DiagnosticReport não é utilizado (os resultados entram diretamente como Observation).

O campo Observation.code é obrigatório (1..1, MustSupport). O binding IPS é de força preferred e aponta para o ValueSet results-laboratory-observations-uv-ips (LOINC). A RNDS utiliza o ValueSet BRTipoObservacao (required — LOINC + SNOMED CT) via BRNomeExame para identificar o nome do exame. O campo Observation.category deve ter o valor fixo laboratory (preferred) conforme o IPS, diferente da RNDS que usa BRCategoriaObservacao/BRCategoriaExame (required).

O campo Observation.specimen deve referenciar o perfil SpecimenBRIPS (obrigatório no perfil RNDS laboratorial; suprimido nos testes rápidos COVID). O campo Observation.method é 1..1 obrigatório no perfil RNDS laboratorial (BRDiagnosticoLaboratorioClinico) mas opcional no IPS. Para resultados qualitativos, o campo value[x] deve usar o binding results-presence-absence-uv-ips (SNOMED CT IPS Free Set).

O documento REL ativo na RNDS é o BRResultadoExameLaboratorial-3.2.1 (Composition). Versões depreciadas do documento REL: 1.0, 1.1 e 2.0 (Retired em 15/03/2024) e BRResultadoExameLaboratorioTesteRapido-1.0. O perfil Observation BRDiagnosticoLaboratorioClinico-1.0 também foi depreciado; o perfil Observation BRDiagnosticoCOVID19-TesteRapido-01.01 segue em uso para resultados de testes rápidos de COVID-19.

Uso indevido

Este perfil não deve ser usado para resultados de Patologia (anatomopatológico) nem de Radiologia/Imagiologia — esses estão fora do escopo do BRIPS por ausência de dados estruturados na RNDS. Não deve ser usado para sinais vitais — use VitalSignsBRIPS. Não deve ser usado como container de diagnóstico (DiagnosticReport não está no escopo do BRIPS).

Extensões

Este perfil não possui extensões nacionais adicionais além das herdadas do IPS.

Limites e Relacionamentos

O recurso ObservationResultsLaboratoryBRIPS é referenciado pela seção “Resultados” (LOINC 30954-2) da CompositionBRIPS (section.entry). Referencia e é referenciado por:

• SpecimenBRIPS — referenciado em specimen (amostra biológica analisada no exame laboratorial).
• PatientBRIPS — referenciado em subject (paciente cujos resultados são descritos).
• PractitionerBRIPS / PractitionerRoleBRIPS / OrganizationBRIPS — referenciados em performer (responsável / laboratório executor).
• device-BRIPS — referenciado em device (equipamento de medição quando aplicável).
• ConditionBRIPS — o perfil RNDS BRCondicaoSaude referencia BRDiagnosticoLaboratorioClinico em stage.assessment (confirmação da suspeita diagnóstica por exame). No IPS, esse elo é representado como Condition.stage.assessment → Reference(DiagnosticReportBRIPS ou ObservationResultsLaboratoryBRIPS).

Condição de entrada

A RNDS utiliza os seguintes perfis baseados no recurso Observation para enviar resultados de exames laboratoriais:

O documento ativo é o BRResultadoExameLaboratorial-3.2.1 (Composition), que utiliza o perfil Observation BRDiagnosticoLaboratorioClinico-3.2.1. Versões anteriores do perfil Observation ainda podem estar presentes na RNDS em registros históricos:

• BRDiagnosticoLaboratorioClinico-1.0 (depreciado) — usado nos REL versões 1.0 e 1.1
• BRDiagnosticoLaboratorioClinico-2.0 (depreciado) — usado no REL versão 2.0; inclui effectiveDateTime
• BRDiagnosticoLaboratorioClinico-3.2.1 (vigente) — usado no REL 3.2.1; inclui a extensão patógeno (BRTerminologiaPatogeno)
• BRDiagnosticoCOVID19-TesteRapido-01.01 — restrição de BRDiagnosticoLaboratorioClinico para diagnóstico rápido de COVID-19 (suprime interpretation e specimen)

O curador do sumário deve processar registros em qualquer dessas versões que ainda estejam armazenados na RNDS, aplicando o mapeamento correspondente à versão identificada. O Mapeamento de Estrutura — Versão 1.0.0 cobre os perfis depreciados (BRDiagnosticoLaboratorioClinico-1.0 e -2.0 e BRDiagnosticoCOVID19-TesteRapido-01.01); o Mapeamento — Versão 1.1.0 cobre o perfil vigente (BRDiagnosticoLaboratorioClinico-3.2.1).

O REL 3.2.1 utiliza o ValueSet BRNomeExame, que importa todos os códigos dos CodeSystems BRNomeExameLOINC e BRNomeExameGAL, excluindo apenas os códigos de sequenciamento genômico do SARS-CoV-2 (ex.: 94764-8, 94822-4, 95424-8, 95609-4, 95970-0, 96741-4, 98062-3). O perfil aceita, portanto, resultados de qualquer exame laboratorial codificado nesses sistemas — não apenas COVID-19 e Monkeypox.

A RNDS não utiliza o elemento language nem apresenta as descrições dos códigos dos ValueSets, que são obrigatórios no IPS — sendo necessário acrescentar esses dois elementos ao modelo estrutural do lado do IPS.

Os exames do último ano a partir da data de solicitação do sumário, informados no RAC, também são incorporados neste recurso.

Condição de saída

Resultados de exames de doenças de notificação compulsória (como COVID-19 e Monkeypox) cujo value.valueCodeableConcept indique resultado positivo — conforme os valores da tabela abaixo — serão exibidos independentemente da data em que foram realizados. Todos os demais resultados de exames do último ano também serão incorporados ao sumário.

Mapeamentos terminológicos RNDS — IPS

Os resultados qualitativos da RNDS utilizam a tabela proprietária BRResultadoQualitativo, que especializa os resultados por categorias (Detectável, Avidez, Presença, Positivo/Negativo etc.). Foi necessário mapear estes conceitos para o ValueSet do IPS ObservationInterpretationCodes (elemento interpretation).

Não foi possível mapear dois tipos de resultado: HISPT e AVIDEZ — nestes casos o elemento interpretation não será preenchido.

Também foi mapeado BRCategoriaExame-1.0 para ObservationCategoryCodes do IPS, pois o CodeSystem da RNDS categoriza exames usando subgrupos do grupo 02 (Procedimentos com finalidade diagnóstica da Tabela SUS), enquanto o IPS lista categorias genéricas de Observation.

Mapeamento de estrutura — Versão 1.0.0

Recurso: Observation

Atualmente a RNDS recebe várias versões do recurso BRDiagnosticoLaboratorioClinico e do BRDiagnosticoCOVID19-TesteRapido e foi necessário realizar o mapeamento de estrutura para todas as versões e seus respectivos elementos Observation, conforme apresentado a seguir.

A RNDS não utiliza o elemento language e nem apresenta as descrições dos códigos dos ValueSets utilizados, que são obrigatórios no IPS. Deste modo foi necessário acrescentar esses dois elementos ao modelo estrutural do lado do IPS.

Mapeamento de estrutura BRDiagnosticoLaboratorioClinico-1.0 para ObservationResultsLaboratoryBRIPS

Este mapeamento utiliza o perfil Observation (BRDiagnosticoLaboratorioClinico-1.0), que foi depreciado. Este perfil referencia o valueset BRCategoriaObservacao, no elemento categoria que não existe mais na RNDS.

No perfil BRDiagnosticoLaboratorioClinico-2.0 o elemento effectiveDateTime foi incluído.

Nessa versão há uma a inclusão da extensão patógeno para informar qual o patógeno encontrado no exame – conforme BRTerminologiaPatogeno. A informação contida na extensão Patógeno será informada no componente value.valueCodeableConcept utilizando a terminologia informada pelo MS.

Mapeamento de estrutura BRDiagnosticoCOVID19-TesteRapido-01.01 para ObservationResultsBRIPS

​O perfil Observation BRDiagnosticoCOVID19-TesteRapido-01.01, uma restrição do recurso BRDiagnosticoLaboratorioClinico-2.0 onde foram suprimidos os elementos interpretation, method e specimen. Esse recurso utiliza o value set , e para o nome de exame, o ValueSet BRNomeExameTRCOVID19LOINC. Todos são mapeados para o mesmo ValueSet do IPS, o Results Laboratory Observation IPS (LOINC), pois esses exames são um subconjunto do mesmo.
